Our partner service is Phayathai 1 Hospital Co.,Ltd (Corporate number 0105544117879, whose registered address is 64/1 Thanon Si Ayutthaya, Thanon Phaya Thai, Ratchathewi, Bangkok 10400.
Ontarget Medicine Ltd., a group of experts in precision medicine, primarily aims to make available tailored oncology solutions for all cancer patients. In addition to providing cutting-edge cancer diagnostic solutions, we seek to deliver the most personalized cancer care to all cancer patients. Understanding that each patient has unique needs and demands customized care solutions, we have worked with leading cancer care providers to develop pioneering digital services for holistic needs assessment (Ontarget Insight™) and for home-based care monitoring (Ontarget Planner™). Careteam at the hospital can assess in real-time the patients’ needs and respond to their requests through our digital environment (Ontarget Dashboard™).
Ontarget Medicine Company Limited are the sole controller of your personal data provided to, or collected by or for, or processed in connection with our healthcare services.
Your relationship is with Ontarget Medicine Co.Ltd. Should you be interested to access your data, Ontarget Medicine Co. Ltd would be the sole entity to which you would make such a request.
Information Access and Collection
We use the following categories of personal data:
Personal details
You must create an Ontarget Planner account in order to use our Service. You can create an account by providing your name, date of birth, physical address and email address. You are responsible for the accuracy of the information that you provide to us.
Health and medical information
Health and medical information is the most common type of information we have about you: details about your health, symptoms, treatments, consultations and sessions, medications, and procedures. This includes information from your medical visits as well as interactions with our digital services, such as 7-day daily question, side-effect check-up, quality-of-life check-up, health monitoring, referral notes, and appointment. Your interactions with our digital services may be shared with our doctors for the objectives of improving your experience and providing you with health care.
When you register with us and use our healthcare services, we receive some of this information directly from you. If you use our services, we may be able to obtain your medical history through hospital records. Your Ontarget Planner health record is updated with any correspondence we get from you.
We may also have data about you and your health from other apps, devices, and services if you have granted us permission to share that data. You might choose to share information collected from a smartwatch or other comparable gadget with our App, for example.
Technical information and analytics
When you use our App, we may gather the following information automatically if your device or browser settings allow it:
technical data, such as the IP address, your login information, OTP, system and operating system platform type and version, device model, browser or app version, time zone setting, language and location preferences, wireless carrier, and your location (based on IP address); and information about your visit (such as when you first used the App and when you last used it, as well as the total number of sessions you've had on that App), including the products and services you viewed or used, App response times and updates, and interaction information (such as button presses or the times and frequency of your interactions with the communications we send to you in the App or otherwise)
We collaborate with companies that provide analytics and advertising services (for our services only and not for third party advertising). This includes assisting us in better understanding how users engage with our services, distributing our advertising on the internet, and tracking the effectiveness of our services and commercials.
Information Stored on Your Mobile Device
With your permission, we may collect information stored on your mobile device, such as photos, or access resources on your mobile device, such as the camera.
Information obtained from third party services
You may choose to sign up with a social media account. If you want to do so, that provider will provide us limited information about you, such as your email address and name. We may also utilize information from other sources, such as specialist firms that offer information, online media outlets, our commercial partners, and public registries, if we are working in compliance with data protection regulations. This data can be used to help us develop and assess the effectiveness of our services.
How We Use Your Information
The purposes for which we use your personal data and the legal grounds on which we do so are as follows:
Providing you a service
· We collect and utilize your personal and financial information in order to create and fulfill our contract with you.
· In order to provide you a service, we obtain and use your medical information including medical diagnosis and the provision of healthcare or treatment. This includes data gathered during our meetings with you (such as notes and recordings), our digital services, and medical history from your prior Phayathai 1 Hospital health record. It may also include exchanging information with other healthcare professionals as needed for you to get care, such as specialist referral services, therapists, pharmacists, hospitals, accident and emergency services, pathology service providers, and diagnostic centers you choose for imaging needs.
Making healthcare accessible
· We will use your medical information (after removing personal identifiers such as your name, address, and contact details) with your consent to improve our healthcare products and services, as well as our artificial intelligence system, so that we can provide better care . Your medical record (both records received and records created by us), transcripts and recordings of your consultations, and your interactions with our artificial intelligence services, such as our symptom checker, may all be included in this medical information.
· We may request your permission to access your location information through our App or browser.
Keeping you up to date
· We use your email address and phone number to send you occasional updates and marketing messages where you have not opted out, You can opt out at any time.
· In order to provide you with high quality preventative and occupational health care services, we may contact you by SMS, email and/or other means.
Other uses
· We may analyze data about the use of our products and services to improve our App and website, forecast demand for service, and understand other trends in use to provide better services.
· Where necessary, we may need to share personal and financial details for the purposes of fraud prevention and detection.
· We also store your medical information, such as notes from consultations, recordings of our consultations with you as well as your interactions with our digital services including interactions with our chatbot, 7 days daily question, sides effect check up, quality of life check up, health monitoring, recommendation notes, and appointment, for safety, regulatory, and compliance purposes. For example, we may need to review your information and, where necessary, make disclosures in compliance with reasonable requests by the regulator as otherwise required by law or regulation.
· Where necessary for safety, regulatory and/or compliance purposes, we may audit consultations and your other interactions with our services. Strict confidentiality and data security provisions will apply at all times to any such audit and access.
How we share your data
We may share your personal data with companies we have hired to provide services on our behalf, including those who act as data processors on our behalf, acting strictly under contract. Those data processors are bound by strict confidentiality and data security provisions, and they can only use your data in the ways specified by us.
Where you access our services through your health insurance provider or any of our commercial partners (including your employer) we may share with such a partner your name, date of birth, email address, policy number, location, and the fact you have registered/used the service (and any other similar information). We will not without your consent share any details relating to the content of your consultation with us or your health/medical records. With your consent, we may share the date of the appointment, details of your diagnosis, prescription, whether or not you had a referral made and other similar information about your appointment with us.
Information sharing with other healthcare providers
· We will share your information with your other health and social care professionals if it is required for your treatment or care. Your Phayathai 1 Hospital and other bodies, for example, expert referral services, therapists, pharmacists, hospitals, accident and emergency services, pathology service providers, diagnosis centers chosen by you for imaging requests, and other health and care bodies. In order to comply with our legal responsibilities, we may share information with such services for the purpose of data security.
How long we retain your data
In compliance with industry standards, we keep your medical records on file. Our retention policy is summarized here, however we may keep records that do not identify you for legitimate business objectives like managing or developing our business, or records for longer periods as required by law or regulation.
Accessing or Modifying Your Information
We do not store your personal health data on your mobile device. We store all your personal health data, including your primary care information, medication information and diagnostic information, on secure servers.
Please carefully use our Service and review your account settings. You are responsible for keeping your password confidential.
Data sent to and from the App is encrypted. We will employ rigorous processes and security mechanisms to prevent unauthorised access. Your personal information is treated securely and in compliance with this policy.
Covid-19
Our App is not Covid-19 patient contact tracing. We do not obtain or store any information about Covid-19.
What rights you have over your data
If you have an account on this site, or have left comments, you can request to receive an exported file of the personal data we hold about you, including any data you have provided to us. You can also request that we erase any personal data we hold about you by emailing us at info@ontargetmed.co. This does not include any data we are obliged to keep for administrative, legal, or security purposes.
Changes to Our Privacy Policy
